Liposuction for lipoedema

Liposuction for lipoedema is a specialist surgical procedure that removes the abnormal fatty tissue characteristic of lipoedema — a chronic condition causing symmetrical, disproportionate fat deposits that do not respond to diet or exercise. Unlike cosmetic liposuction, lipoedema liposuction targets diseased adipose tissue while aiming to preserve lymphatic vessels, aiming to reduce pain, improve mobility, and restore limb proportion. At the Great North Lymphatic Centre, we assess and treat lipoedema within a dedicated surgical pathway led by Mr Chad Chang and Mr John Henton.

01What is lipoedema?

Lipoedema is a chronic condition involving abnormal fat deposition, most commonly in the legs and sometimes the arms. It is almost exclusively diagnosed in women. The fat is characteristically symmetrical, painful to touch, and resistant to diet and exercise. It tends to progress over time, causing heaviness, easy bruising, and difficulty with mobility.

Lipoedema is frequently misdiagnosed as obesity or lymphoedema. Many patients spend years being told to lose weight before receiving a correct diagnosis. Accurate assessment is the essential first step before any treatment decision.

02How is lipoedema different from lymphoedema?

Lipoedema is a disorder of fat tissue. Lymphoedema is a disorder of lymphatic drainage. They are different conditions with different causes, though they can overlap — a situation known as lipo-lymphoedema, where long-standing lipoedema eventually damages lymphatic vessels and causes secondary lymphoedema.

This distinction matters because the surgical approach differs. In lipoedema, the primary goal is to remove abnormal fat. In lymphoedema, the primary goal is to restore or bypass lymphatic drainage. When both conditions are present, the surgical plan must account for both.

At the Great North Lymphatic Centre, ICG lymphography is available as part of our assessment where indicated. It can help determine whether lymphatic function is intact, impaired, or whether lipo-lymphoedema is present. This guides surgical planning and protects remaining lymphatic pathways during liposuction.

03How does lipoedema liposuction differ from cosmetic liposuction?

Cosmetic liposuction reshapes normal fatty tissue for aesthetic purposes. Lipoedema liposuction removes diseased adipose tissue to reduce pain, heaviness, and functional limitation.

The differences in practice:

  • Tissue type: Lipoedema fat is often more fibrous and vascular than normal fat, requiring careful technique to minimise bleeding, bruising and injury to lymphatics.
  • Goal: Symptom relief and functional improvement, not solely cosmetic reshaping.
  • Lymphatic awareness: Where indicated, we use ICG imaging to identify and protect remaining lymphatic vessels during the procedure, reducing the risk of secondary lymphoedema.
  • Volume: Lipoedema liposuction often involves larger volumes than cosmetic procedures.
  • Post-operative care: Compression and follow-up are managed within a specialist pathway, not a cosmetic aftercare programme.

04Who is suitable for lipoedema liposuction?

You may be suitable if:

  • You have a confirmed or suspected diagnosis of lipoedema.
  • Your symptoms — pain, heaviness, disproportionate limb size, easy bruising — affect your daily life, mobility, or wellbeing.
  • Conservative measures such as compression, manual lymphatic drainage, and exercise have not adequately controlled your symptoms.
  • You are medically fit for surgery and anaesthesia.

Lipoedema liposuction may not be appropriate if your symptoms are primarily caused by lymphoedema rather than lipoedema, or if there are medical reasons that make surgery unsafe. This is why specialist assessment, including imaging where indicated, is important before any surgical decision.

05What does the assessment involve?

At the Great North Lymphatic Centre, your assessment typically includes:

  • Clinical history: A detailed review of your symptoms, how they have progressed, previous treatments, and any family history of lipoedema.
  • Physical examination: Assessment of limb proportion, tissue quality, tenderness, and distribution of fat deposits.
  • ICG lymphography: Where your assessment suggests it would be helpful, we can use ICG imaging to assess lymphatic function. This helps us distinguish pure lipoedema from lipo-lymphoedema and plan surgery that protects your lymphatic system.
  • Discussion: We explain what surgery can realistically achieve, the recovery process, and any ongoing care you would need afterwards.

What to expect

01On the day of surgery

Most practical steps on the day of surgery are described on our Patient Information page.

  • For lipoedema liposuction, surgery duration depends on the areas and volume being treated, typically ranging from 2 to 3 hours.
  • We use power-assisted tumescent liposuction. Tumescent fluid is infiltrated into the treatment areas to minimise bleeding and provide local anaesthesia to the tissues. A powered cannula then removes the abnormal fat in a controlled, even pattern through small incisions (usually less than 1 cm) placed at intervals across the treatment area.
  • At the end of the procedure, we apply compression garments or bandaging.
  • Most patients go home the same day or after one night of observation.

02After surgery

  • Bruising, swelling, and some fluid leakage from incision sites are normal in the first days.
  • We apply compression garments immediately after surgery. You will need to wear these consistently during the recovery period, and we will advise you on the timeline.
  • Gentle walking is encouraged early to reduce stiffness and clot risk. Avoid heavy lifting and high-impact exercise for several weeks.
  • Return to work depends on the extent of surgery and your role. We provide individualised guidance and fit notes.

03Follow-up, timelines and risks

  • We typically review wounds at regular intervals, with further follow-up arranged based on your progress.
  • Limb size reduction is often visible immediately, with further improvement as post-operative swelling settles over the following weeks and months.
  • Pain, heaviness, and tenderness typically improve progressively. Many patients describe a significant improvement in comfort and mobility.
  • Final results depend on the extent of disease, the volume removed, and your ongoing self-care.

01Will liposuction cure my lipoedema?

Liposuction removes abnormal fat tissue and can significantly reduce symptoms, but lipoedema is a chronic condition. While many patients experience lasting improvement in pain, proportion, and mobility, new fat deposition can occur over time, particularly with hormonal changes or significant weight gain. Healthy lifestyle measures and follow-up remain important after surgery.

02How many areas can be treated at once?

This depends on your individual anatomy, the extent of disease, and what can be safely treated in a single session. Some patients have all affected areas treated in one procedure; others benefit from a staged approach. We discuss this during your assessment and agree on a plan that balances thoroughness with safety.

03Is lipoedema liposuction available on the NHS?

Lipoedema liposuction is not currently available on the NHS. Treatment is accessed through private healthcare. At the Great North Lymphatic Centre, we offer lipoedema assessment and surgery as a private service at The Beverley in Gateshead.

04What about compression after surgery?

Compression is mandatory in the immediate post-operative period to support healing, reduce swelling, and maintain your results. A typical plan includes:

  • Firm garments or bandaging immediately after surgery.
  • Consistent wear throughout the initial recovery period.
  • Ongoing compression is recommended for continued support where possible, to help maintain limb size and comfort long term.

05Is this the same as cosmetic liposuction?

No. Lipoedema liposuction treats a medical condition using specialist techniques that account for the unique properties of lipoedema tissue and the importance of preserving lymphatic function. It is not cosmetic reshaping of normal fat.
